Active Care Nursing Agency Pty Ltd is an association dedicated to providing customer focused disability support services that are tailored to your individual needs.

Established in March 2020 by Sandy, in response to the lack of suitable organisations available to support individuals wanting to live in their own homes, in a way they desired.

As a team we do not only have the experience and knowledge to be able to deliver a high level of professional services, but we ensure we are operating with the utmost integrity, respect and best interests of all of our clients - at all times. Our director Sandy has over 48 years of experience, from intensive care nursing, oncology nursing and many years of nursing management under her belt that enables her to plan and manage all facets of nursing care services on an individualised basis.

It is incredibly important to us that you/your primary caregiver/representative are actively involved in the care-planning phase to assist in the smooth transition and longevity of your care journey.

The services you are receiving from us here at Active Care Nursing Agency are being delivered by an organisation that has experience in all areas, from Management through to Customer Service.

Active Care Nursing Agency Pty Ltd focuses on advancing the lives of those with a disability by supplying our clients with adaptable care services that will enhance their lives.

We do this by ensuring our services are designed for each individual and their needs, keeping focused on positive outcomes.

We aim to be a leading supplier of individual care, the most trusted and respected service provider.

We have established a culture that trains and supports our team members so they can provide exceptional service to our clients. 

CUSTOMER RIGHTS & RESPONSIBLITIES

The customer or, with their permission their carer, has access to all information about themselves held by Disabled and Carers Association Inc.

In cases where a customer has a legal guardian or advocate appointed to act on their behalf, the rights of the guardian or advocate are to be acknowledged and respected to the extent stipulated in the guardianship or advocacy arrangements.

Customers’ access to services should be decided only on the basis of need and the capacity of the service to meet that need.

Customers have the right to refuse a service and refusal should not prejudice their future access to services.

Customers have a right to complain about the service they are receiving without fear of losing the service or any other retribution.

Complaints by customers should be dealt with fairly, promptly and without retribution. The customer may involve an advocate of their choice to represent his/her interests.

Customers’ views should be taken into account in the planning of service delivery.

Customers’ rights to privacy and confidentiality shall be respected. You can expect that no information about you will be provided to anyone outside Disabled and Carers Association Inc without your permission.

Services should be provided in a safe manner, which respects the dignity and independence of the customer, is responsive to the social, cultural and physical needs of the customer and also the needs of the carer.

Customers should be actively involved in the development of their service delivery or care plan.

Customers should play their part in helping Active Care Nursing Agency to provide them with
services.

Customers should advise Active Care Nursing Agency promptly of any changes in their care
needs.

Customers should provide reasonable notice if service is not required.

Customers should act in a way which respects the rights of Active Care Nursing Agency staff and contractors.

Customers should assist Active Care Nursing Agency by providing a safe work environment for Active Care Nursing Agency staff and contractors.

Customers should advise Active Care Nursing Agency if they are unhappy with any aspect of the service they are receiving, in order that Active Care Nursing Agency may be able to rectify the problem.

Customers should take responsibility for the results of any decisions they make.
